Forms

All required forms can be found on your Medicat Patient Portal which is access through your WOU student portal. Many of them are fillable and will automatically upload into your electronic health record.

Required Documents Prior to Attending WOU:

All incoming domestic students must submit a Health History form, immunization records (specifically for MMR vaccinations), and proof of COVID-19 vaccine. For either the MMR or COVID-19 vaccination, there are separate exemption forms for each. The exemption form for MMR is attached to the Health History form. This information is required by law and or mandated by WOU for students to attend WOU and failure to provide this information will result in a registration hold.

Other Forms:

ROI  – Release of Information form. To be completed if a student requests records to be released TO or FROM SHCC.

COVID-19 Vaccine Screening and Consent Form – to be completed prior to COVID-19 vaccine. Bring the form with you to SHCC at the time of your appointment.
